Our Facility
We are proud to provide a state-of-the-art facility for the highest quality dental care available. One of our top priorities is to protect the well-being of our valued patients. For this reason, our office meets and surpasses all OSHA (Occupational Safety and Health Administration) and CDC (Center for Disease Control) standards.
Our office is located on the Concord Hospital Campus, in the Pillsbury Medical Building. This makes it very convenient for our patients to receive both their medical and dental care in one location.
New Patient Pre-Registration Process
So that we can be completely ready for your arrival, we ask all new patients to complete the necessary new patient registration, dental & medical history and dental insurance information forms ahead of time. For your convenience, we have made all of the new patient forms available to you on this website. Simply click on "Our Practice", select and print out a copy of each form. You can then mail or fax your completed registration and information forms to us. We will review everything you provide and call you to set up your first visit to our office.
Appointments
If you would like to make an appointment, please contact our office by phone or email. Our office facilitates communications and emergency calls and appointments are available and welcome, especially for new patients seeking help.
Financial
We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. For patients who require major work, a complete payment plan is designed with an appropriate payment schedule. Forms of payment accepted by the office include any combination of: cash, checks and/or all major credit cards.
Changing or Postponing Appointments
We know that unexpected events can happen that will require plans to change. If you are unable to keep an appointment you previously reserved with us, we ask that you kindly provide us with a 2-business day (at least 48 hours) notice if at all possible. We ask for this advance notice so that we have enough time to offer this appointment to another patient. A room set-up fee may be charged if a patient does not show up for an appointment without sufficient notice.
