Our Facility
We are proud to provide a state-of-the-art facility for the highest quality dental care available. One of our top priorities is to protect the well-being of our valued patients. For this reason, our office meets and surpasses all OSHA (Occupational Safety and Health Administration) and CDC (Center for Disease Control) standards.
Our office is located on the Concord Hospital Campus, in the Pillsbury Medical Building. This makes it very convenient for our patients to receive both their medical and dental care in one location.
New Patient Pre-Registration Process
So that we can be completely ready for your arrival, we ask all new patients to complete the necessary new patient registration, dental & medical history and dental insurance information forms prior to their scheduled first visit. For your convenience, we have made all of the new patient forms available to you on this website. Simply click on "Our Practice", select and print out a copy of each form. You can then mail or fax your completed registration and information forms to us. We will review everything you provide and call you if we have any questions.
Free Dental Plan Evaluations
We have found that the majority of our new patients know very little about their dental plan benefits. As a courtesy (and so that there are no "out-of-pocket surprises") part of our new patient intake process is to contact your dental plan insurer to determine what your plan deductibles, reimbursement percentages and benefit limitations may be to help you understand your cost-sharing expectations in advance of receiving any necessary dental care.
Appointments
Our goal is to set up an initial visit for all new patients at the time of first contact and then follow up with the necessary patient registration forms. If you would like to make an appointment, please contact our office by phone, fax or email. Although we are not an "urgent care dental center", emergency appointments are available and welcome, especially for new patients seeking help.
Dental & Financial Plans
We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. For patients who require major work, a complete payment plan is designed with an appropriate payment schedule. Forms of payment accepted by the office include any combination of: cash, checks and/or all major credit cards. We can even set up a "lay-away plan" for those who would like to pay small installments ahead of time in order to build a personal dental spending account for upcoming care!
Changing or Postponing Appointments
We know that unexpected events may happen that will require dental care plans to change. If you are unable to keep an appointment you previously reserved with us, we ask that you kindly provide us with a 2-business day notice (at least 48 hours) when possible. We ask for this advance notice so that we have enough time to offer this appointment to another patient. A room set-up fee may be charged if a patient does not show up for an appointment without sufficient notice.
